Maintaining a decision ledger seems like it should be mandatory, yet many organizations fail to do so and instead sift through endless sets meeting minute records.  While meeting minutes are the official records of decision-making, they need not be the only reference point to identify prior decisions, spending authorizations, etc. 

Solving the continuity conundrum is one of the biggest challenges facing common interest communities.  How do you record all the information that your successor needs to know?  Keep a decision ledger and integrate action items!
