Policies, Procedures and Resolutions (PPR) form the third leg of the governing documents triumvirate.

Maintaining a decision ledger seems like it should be mandatory, yet many organizations fail to do so and instead sift through dozens of individual meeting minute records. Meeting minutes are the official records of decision-making, but they need not be the only reference point to identify prior decisions, spending authorizations, etc.
